About this page: This page contains two different calculators for post-pubertal testicular germ-cell tumors. The first works through the AJCC stage. The second assigns the classic IGCCCG good/intermediate/poor prognosis group used when first-line chemotherapy is being planned. These tools explain classification rules; they do not choose treatment. They are not designed for lymphoma, sex-cord/stromal tumors, prepubertal-type teratoma or other non-germ-cell testicular tumors.

AJCC Stage Calculator

This calculator is designed for the usual situation after orchiectomy. Use the pT category from the pathology report if you know it, the lymph-node information from imaging or RPLND, the distant-metastasis information from imaging, and the post-orchiectomy tumor-marker results used by the treating team for staging. If you know you have an invasive germ-cell cancer but do not know the exact pT category, the calculator can still give the broader stage when the other information allows it.

Which blood draw? For the usual post-orchiectomy AJCC stage, use the post-orchiectomy tumor-marker results your doctor considers appropriate for staging. If the first postoperative AFP or beta-hCG is still falling, the first draw may not be the final staging value. The lowest values reached by normalization or plateau before additional treatment, or before a value starts rising again, are used.

What the calculator cannot do: It cannot decide whether a suspicious scan finding is really cancer, whether an unusual tumor-marker result is a false positive, or which treatment is best. If the calculator disagrees with the stage given by the treating team, use the treating team's stage and ask how it was determined.

Classic IGCCCG Prognosis Group Calculator

This calculator assigns the classic good, intermediate or poor prognosis group used when planning first-line chemotherapy for metastatic or marker-positive germ-cell cancer. It does not apply to relapse after prior cisplatin chemotherapy.

For this calculator, use tumor-marker values measured immediately before starting first-line chemotherapy for metastatic disease. These are not necessarily the same blood-test results used for the post-orchiectomy AJCC stage.

If the result is intermediate or poor prognosis: These cases are less common and more complicated. Current EAU guidance reports better outcomes for intermediate- and poor-prognosis patients treated at high-volume centers, and recommends interdisciplinary germ-cell-tumor expertise for poor-prognosis disease whenever possible.
There is also a newer, more individualized IGCCCG model for nonseminoma. The official IGCCCG Update model adds age, lung metastases and more detailed marker information and estimates 3-year progression-free survival. In plain English, that is the estimated chance of being alive without the cancer having progressed or returned three years after starting first-line chemotherapy. It is not the same thing as overall survival or a literal chance of cure. The classic group above is still used for treatment decisions. Open the official IGCCCG Update calculator.
